New patient intake process

Overview

I work with a small number of patients at any one time in order to provide thoughtful, individualised care. To ensure appointments are safe, clinically appropriate, and a good fit for my model of care, all prospective patients undergo a new patient intake process to determine their suitability. Please note that I receive a large volume of referrals and, regrettably, can only accept a small proportion of them.

The screening and intake process is the same for all new patients, even if there has been a prior diagnosis or management from other healthcare providers. This is because I am legally responsible for my prescribing decisions and must conduct a comprehensive assessment before I can prescribe.

I do not accept referrals for workers' compensation, insurance claims, legal claims, family court matters, or DVA-funded care. I don't prescribe Xanax (alprazolam) or medicinal cannabis.

Here's what the screening and intake process involves: â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹

What to expect at the appointment

Your initial telehealth consultation will last 60-90 minutes via video call. Please join the waiting room 5-10 minutes early to ensure your camera, audio, and internet connection are working.

Assessment

During this session, I will conduct a comprehensive psychiatric assessment covering:

- Current symptoms and their impact on your daily life

- Medical conditions, physical symptoms, and allergies

- Current and previous treatments

- Family history of health conditions

- Substance use history

- Developmental history

- History of any major/difficult life events

- Your goals and preferences

If we run out of time, we can address further details in a follow-up appointment.

Formulation

After gathering information, I will share my impressions, including your diagnosis and treatment plan. I'll explain key concepts and patterns during our discussion.

Plan

The final step involves establishing an investigation, treatment and follow-up plan, which may include:

- Investigations: Blood tests, ECGs, sleep studies, questionnaires, and, for ADHD assessments, input from someone close to you.

- Treatments: Medication adjustments (excluding stimulants at the initial consultation), psychological therapy, and simple strategies to manage your symptoms.

- Organise your next appointment.

After the initial consultation

After your initial consultation, you will receive:

- A summary letter to your GP

- A written overview of your plan

- Resources to help you understand and manage your condition (if relevant)

- Investigation request forms (if necessary)

- Links to symptom questionnaires (if necessary)

- Access to the patient portal for document sharing and online bookings.

Appointment schedule

Appointment frequency

After attending an initial consultation, patients typically require follow-up appointments. It is important to follow up regularly early in your treatment journey, with appointments becoming less frequent as you progress.

This approach ensures your treatment is adjusted appropriately, and any issues or concerns are identified and managed early. During this stage of your care, you'll learn to manage your condition(s) effectively, aiming to achieve stability as soon as possible.

The exact timing depends on the diagnosis being managed. Below is the typical schedule; more frequent appointments are available if needed. All follow-up appointments are booked as 60-minute sessions to ensure you have the time you need, but there is no obligation to use the full hour. The fee is determined after each follow-up consultation, as outlined on the Fee Schedule page. â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹â€‹

ADHD appointment schedule

For ADHD management, the minimum appointment schedule is as follows:

  • Appointment 1: This is the initial consultation, which is described above

  • Appointment 2: This is usually scheduled 2-4 weeks after the initial consultation. We finalise the assessment and review your results. If appropriate, stimulant medication is usually commenced at this appointment. This appointment can be scheduled once all investigations are completed.

  • Appointment 3: This is usually scheduled 4 weeks after your second appointment. We'll review the outcome of your stimulant trial and adjust the plan further.

  • Appointment 4: The timing of this appointment will be determined at appointment 3. It is usually scheduled 3 months later if all is going well. If further adjustments were needed at appointment 3, follow-up will be scheduled every 4-6 weeks until the plan stabilises, then you'll schedule your 3-month follow-up.

  • Subsequent appointments: If you are doing well, you'll attend two 6-month reviews, then a 12-month review. If your mental state and treatment plan remain stable at this stage, GP shared care can be initiated, with follow-up after two years on the first occasion, then three-yearly ongoing.

  • Long term: Generally, ongoing care is required long-term due to the current stimulant prescribing code, which requires psychiatrist reviews at least once every three years for patients eligible for shared care.

Appointment schedule - other mental health conditions

For other conditions, and for ADHD patients with complex co-occurring conditions and/or additional support needs, the frequency of follow-up appointments is determined by clinical need as well as your personal circumstances and preferences.

Follow-up appointments can initially be scheduled weekly, up to 3 monthly, gradually reducing the frequency as your mental state and treatment plan stabilise. Some patients require long-term ongoing care, while others can be discharged once stabilised.
